Hi everyone,
Please excuse the cross posting, but I'm trying to get as much
information as possible on this one.
I'm currently working on a project to help increase the
accessibility of Access to Work (AtW) documents. To this end, I've
recently received a set of AtW application and claim forms in Word
format. These are all a bit noddy, but are fairly accessible. I'm
hoping to be able to make them available to everyone in the near future,
subject to Jobcentre Plus approval.
One form which is not included in my set is the Support Worker diary
sheet, which is regularly used in some regions. The documents I have
already have come from the AtW Policy Unit, who do not have an
accessible version of the diary sheet. I need to establish in which
regions this sheet is in regular, continuous use, in which regions it is
in occasional use and in which regions it is not used at all. I know
that it is required with every support worker claim in the West
Midlands. I also know that it is not in regular use in the South-East
region. I've got some ideas about some of the others and can probably
clarify these with AtW advisors in those regions, but I'd like to hear
from AtW clients, since you are at the sharp end. Does anyone have the
diary sheet in an accessible form?

will be that we can make an accessible version of all forms available to
everyone.
