Most of the visual impairment solutions do not prioritise format especially 
because this can have negative consequences regarding readability.  However, 
saving outside their special format into say MS Word will get you some way 
there.  Something like FineReader or OmniPage are your best bet though as they 
will prioritise reproduction of original format.

Hello,

Does anyone know of a program which will scan a document (form for example) and 
keep its format?

A lot of the other scanning OCR software seems to reformat the document and 
changes the layout.
