Well, to LEO I would venture the guess that Lambda-4S would be the smallest
launch vehicle flown. As for escape, the Aerobee escape shot in 1957 is the
smallest I know of, but not exactly a viable method for active satellites
if you ask me.

Very interesting question, rarely replied: which is at present the
smallest launcher to orbit or to escape, solid or liquid, SSTO or
multistage or whatever.
Like: a tiny GH2 gas gun or railgun launched multistage rocket in an
ablative sabot.
