When is a spelling mistake a grammatical mistake (eg. lived/lift) and when is it a semantic/lexical mistake such as you listed below as a accent issue. If I say 'He lives' when I mean 'He leaves' I have changed the meaning. There is a serious problem is granting 'hitamut mi-sh'giyot k'tiv' when what a student writes completely changes the meaning.
