Thank you all for your responses. ONE thing is clear- this line is unclear! Many of you sent me excerpts from literary sources and many of them gave conflicting explanations as to whose "hand" and whose "heart:. I guess these lines are definitely open to interpretation! This is the clearest explanation in my opinion: "The meaning is that the sculptor read Ozymandius' passions so well that they are still shown on the carved face, even though the (sculptor's) hand, that carved them in mockery, and Ozymandias' heart, that held the emotions which caused the "wrinkled lip" and "sneer of cold command", are long since dead.
