As your school's inspector, I wish to make it clear that the instructions for 
testing the oral bagrut are posted on the OFFICIAL MINISTRY OF EDUCATION site. 
I suggest you take a good look at the site for an official answer to which I 
hold my teachers responsible.

This is a good time to remind all of my testers - you are not to sign the 
official form 9540 IF the school grade has not been filled in. Under no 
circumstances should you agree to sign a blank document!!!!! In such a case, do 
not leave the grades with the school, it will be their responsibility to bring 
you the forms in order for you to hand in the grades.
