Thank you to Gloria for helping out here.
I've just discovered that you can avoid phoning the ministry and get
the form you need at this web page:
http://cms.education.gov.il/EducationCMS/Units/CoachAdam/Tfasim/Scanned/ishurpirsum.htm
Fill it in and send copy of the publication to the Ministry, and hey
presto, they promise to deal with it 'soon'!
If you don't get your salary from the Ministry, you need an ishur from
your school that you really are a teacher!!!
