IMNAL - but if commercial usage is allowed then anyone can sell the content.
If they do so, will they give royalty to author?
I don't think so, unless such a clause is stipulated in the CC lic. text.
Can they change the license of the printed book to - all rights reserved -
some thing else?
The share alike would have to be acknowledged otherwise the publisher
would be violating the original lic.
It is best to consult a good IP lawyer; they understand the nuance of
the lic. language and interpret them from a legal stand point.

I got a contract to become a solution partner with a company. The
company sent the template. I did not see anything insidious in the
content but my lawyer pointed out sections which could be used against
me in case of litigation. She proposed amendments that were equitable
to both parties. The company's solicitor firm refused to amend. I
did not sign up.
