As long as you are using 2k8 (possibly 2k5 as well) it should work
okay for the most part.  There are a few areas that don't function
well, such as the project and class wizards, but the basic editor
works well.  2k3 (and I am guessing 2k2) don't work well at all, but
you shouldn't have an express of either of those.  I am not sure at
all about 2k10 though.

Check the Speech and Sounds manager and ensure you have one of the VS
profiles selected.  I also like setting up an all caps and single caps
voice for use in such contexts, with all punctuation spoken and caps
not ignored.

I recently switched from express to standard and did have problems
after installing service pack 1.  Those went away after I started
using the experimental hive enabled by the visual studio integration
pack.  I'm not sure if either of those packages/concerns apply to the
express editions however.

I also don't see anything in the INS+V dialog that obviously applies.

You could always try the replacement scripts from empowerment zone.
Personally using vs2k8 I've been happy with the default set.  Perhaps
if I were doing .NET where trying to create UI mattered it would be
different.
