Google is your friend, John!

https://www.gov.uk/financial-help-disabled/vat-relief

where it says, "You'll need to confirm in writing that you meet these
conditions. Your supplier may give you a form for this."

If the supplier had half a brain, they would know these regulations.

George

Subject: [access-uk] Relief From VAT for White Canes

Hello list,

I needed to buy a new white cane recently.
I first visited the RNIB in Judd Street but found that the canes they were
selling were not to my liking.
The lady behind the counter was kind enough to look on-line for me to see if
there is another supplier that I could use.
She pointed me to Amazon where they have a third party seller who has
exactly what I wanted.
I placed an order with them.
As white canes are eligible for VAT relieve I now need to send the seller a
certificate to this effect.

Does anybody on the list know where I can download a suitable template
please?

Thank you.

Regards, John.
