I did find that page with google. 
It is the suppliers responsibility to supply a form to their customers.
Amazon will not help as it is a third party seller and not Amazon directly.

However I think the supplier does have half a brain.
They first told me how to get a printable invoice, which was not the
question I asked.
They then understood what I wanted and told me to send in the certificate.
I have now asked them to send me a certificate for me to complete and send
back to them.

The company is call "24/7 direct", but it would look as if that name is very
much a misnomer.

I am not sure what they will try to send me.

Hello list,

I needed to buy a new white cane recently.
I first visited the RNIB in Judd Street but found that the canes they were
selling were not to my liking.
The lady behind the counter was kind enough to look on-line for me to see if
there is another supplier that I could use.
She pointed me to Amazon where they have a third party seller who has
exactly what I wanted.
I placed an order with them.
As white canes are eligible for VAT relieve I now need to send the seller a
certificate to this effect.

Does anybody on the list know where I can download a suitable template
please?
