WA Taking Back Transport Forum - Survey Closes 1st August 2014
To prepare for our upcoming Taking Back Transport Forum, (see event details
below) we would be very appreciative if you could take 10mins and complete this
survey. The survey is for WA participants. Please share this survey amongst any
other WA groups. Closes 1st August 2014.

This survey aims to identify broad areas of concern relating to public
transport issues in Western Australia for people living with disabilities. The
survey has been designed in consultation with the Self Advocacy and Peer
Support WA group, and is a preliminary survey for a seminar on this topic. The
comments areas on the survey are designed to be brief, as you will have the
opportunity to provide more detail at the upcoming seminar.
The questions in this survey relate to the person living with the disability.
If you are completing this survey on behalf of another person, remember to
answer all the questions relating to them.

dietary requirements.
The forum is in the Northbridge Piazza Community Centre. Access is behind the
big screen at Northbridge Piazza.
