I do not require the form as I never find myself in a situation like you Hussein having to reclaim VAT as it is always done at the point of purchase. However, I am interested to know from where you got the generic Form.

Hi, Just to tidy up the thread by saying this issue is now resolved. I'm getting the generic form which I should have in the next day or so which will be honoured for any qualifying claims.
