Your XSL has looked good so far to me after scanning through it.

As a general rule, we should avoid having content within <footref>,
although I would prefer to keep the capability there in the DTD. The
current XSL and your proposal for <footref> both take the possibility
of content into account and place the link after that rendered
content.
