As for a text version, not absolutely sure what's meant here, but if that were a propper HTML form then, as far as I'm concerned, that would be better still. Still, that doesn't get you around the need for validation of your initial application as Social Services or a professional of some brand or other has to be found to confirm you are registered blind or other valid criterea you might meet. Wouldn't it be nice if we could be given some nationally recognized ID of being registered. Or would it be nice? Administrative and civil liberties issues accepted, it would be convenient if various concessions could be cleared in that way. Too complicated I guess.
