Hello Kevin,
Thanks for letting me know that, I had no idea that's how Apple implemented
that.  I guess that makes sense since VO will still need to access the
screen to know where the buttons are.  One way to save battery life then
would be to go into your iPhone Settings / Brightness & Wallpaper and turn
the Brightness to the minimum.

Turning on the screen curtain does not save battery. It merely switches all
the LCD shutters to block light but the light is still there.
It's for security, not battery life.
