[projectaon] Some dabbling with 01HH + misc files

I hope I haven't done anything that collides with what someone else is
editing at the moment, but in a burst of unexpected time and energy I
have committed some things related to Highway Holocaust to the CVS
repository, before they got too obsolete.
- Added jdbiofw.inc (Joe bio for the FW books - could be improved)
- Added mgbiofw.inc (Mel Grant bio for 01HH - incomplete placeholder!)
- Updated geninc.mod with the above bios
- Updated genlinc.mod with link to Mel's website
- Updated 01hh.xml to use the above stuff, and with some other changes
to the title and acknowledgements sections. I also wanted to be able
to build the book without complaints, so I fixed some other things
related to well-formedness and DTD compliance (missing footnote
anchors, <signpost> not allowed within <p>, <ul> not allowed within
<p>, and some other tiny detail). Sorry for stepping on your toes
Iain!
Now I shall try to stay away from the FW books and start working on
the LW refresh. And playtest 17tdoi when that time comes - maybe it is
finished enough to start playtesting already?
