I think the Kalkoth battles are fine as they are. A round of combat doesn't necessarily represent one swing from you, followed by one swing from the enemy; it simply represents (in my opinion) how Endurance points are lost over one "time unit" of unspecified length. Both your and the enemy's weapons might have made contact many times in a round. For example, if a Kalkoth has 10 Endurance points left, and in the next round you deal 10 damage to it while it deals 2 damage to you, that result could mean that the Kalkoth bites you for 2 damage after you've hit it with your sword for only 5 damage (but before you hit it for the other 5).
