Most of the last hour has been spent on Books 8-16 replacing the <a ...> footnote markers with <footref ...> elements. That was excruciatingly boring, but at least it now appears to be done. You might want to find an editor like emacs, vim, notepad++, etc. with a regular expression search and replace for occasions like this. Of course that requires learning regular expressions, and as the famous JWZ quote goes "Some people, when confronted with a problem, think 'I know, I'll use regular expressions.' Now they have two problems.".
I am very slowly working out how to use regular expression for /searches/, but using regular expressions for search/replace seems too prone to potential bodging. I think with a regexp find/replace I'd have ended up with much more than just TWO problems! ;-)
Hopefully, there should be no instances of footnote anchors using the old <a ...> style any more. I think you already fixed Books 1-7, Jon, and Books 17+ use the new style anyway. So 8-16 should /in theory/ be fixed as far as this goes.
As with many things, I hadn't intended to look at this, but I spotted it when searching for certain things reported in the error report (i.e. discard vs. shoulder) and decided it needed fixing so that it wouldn't keep distracting me.
